If you are aged 16 or over and would like to be able to make appointments on-line or request your repeat prescriptions you will need to come to register in person at either surgery reception and bring the following documents with you
1. a current form of photo ID eg driving licence or passport
PLUS
2. one utility bill or mobile phone bill or a bank statement bearing your name / family name and address
Ask the receptionist to register you for online
prescriptions and appointments.
If you have children under 12 who live at the same address
as yourself, you will need to bring the above documents to
register for proxy access for online services.
Children 12-15 years can have their own access.
Parental/carer access can ONLY be provided with the prior consent of the 12-15 year old.
