Please call at reception at either Silsden or Steeton surgery to collect your new patient pack which includes your registration form, consent form to share records, a brief questionnaire and a copy of our practice leaflet.

Please bring to the surgery 2 forms of identification when you come to register with us; 1 photograph ID and 1 address confirmation (utility bill within last 3 months).
